IMPORTANT MESSAGE FROM THE SCHOOL NURSE

Welcome all to a new school year at Sacred Heart School.

I ask that all parents/guardians please review this email and submit all documents that apply to their child as soon as possible.
Please note that if your child is a returning student and their grade is not mentioned below, then their file should be up to date and no other documentation is necessary unless otherwise requested in by the nurse.


All emergency cards should have been completed in full and returned to school.

Physicals are required by the first day of school for:

All Kindergarten students
All new/transfer students in Grades 1 through 8

A blank Physical form may be downloaded from the School Nurse’s staff page.


Up to Date Vaccinations are required by the first day of school for:

All New Pre-K students (Flu vaccine is also mandatory for Pre-K 3 and Pre-K 4 by             December)
All Kindergarten students
All new/transfer students Grades 1 through 8
All students in 6th Grade (Tdap and Meningococcal vaccines are required)

Students who don’t meet the Physical and Vaccine requirement will be excluded from school until those documents are received. Please be aware that there requirements also apply to those students who are enrolled in remote learning.

Medications:
No medications (prescription or over the counter) may be sent into school without a medication authorization form completed and signed by pediatrician and parent. Please also check the expiration date of the medication before sending it into school. The medication must have a pharmacy label with the child’s name if it is a prescription, otherwise label the original bottle/box with student’s name/grade if it is an over the counter medication. Medications should be sent into the main office.

Absences:
If your child will not be attending school, a call must be made to the main office to report their absence.
Please make sure that the emergency contact information we have on file for your child is current.

Please respond to calls/written requests sent by the nurse, teacher or administrator as soon as possible.
Please make sure all paperwork/notes/envelopes sent into school have your child’s name and homeroom written on them.

Covid-19 daily entrance screening:
Please refer to our school’s reopening plan posted on the school’s website. Please review this with your child/children so they are aware of the procedures.

If not using FEVER FREE APP, the Covid 19 screening form must be completed each morning and handed to staff at check in where a temperature check will also be performed. Please refer to the “Thursday folder” on school’s website.

School Nurse’s Greeting and Reminders

As the first day of the 2020-2021 school year approaches, I would like to welcome all our returning students as well as all our new students and their families. Needless to say this will be a beginning of a school year like no other we have ever experienced. The faculty, staff and administration at Sacred Heart School are committed to making this transition back into school a safe, pleasant and memorable experience for our students. There are many new guidelines for us all to follow and I would like to remind you of some important requirements/policies.
